Ascend Hospice’s team of professionals will provide you or your loved one with exceptional medical care as well as strong emotional and spiritual support.Care and comfort can be provided in your home, an assisted living or in a skilled nursing facility. Our team focuses on managing any symptoms and pain you may have. We know that patients prefer to stay home and we work to prevent emergency room visits and rehospitalizations which can be stressful and often times avoidable.

If you have questions like 'is now the right time’ or if you wonder ‘when will be the right time”, you can call and speak to one of our clinical directors. Once you are ready your team members of nurses, your physician or our medical director, and our chaplains, social workers, and aides will be with you and your family on every part of your journey insuring that your care and comfort are our first priority. When you or a loved one is ready, our staff will work with your physician(s) to coordinate with you a smooth transition to start care and comfort measures.
