Who Can Benefit From Ascend’s Palliative Care?
Palliative care helps individuals cope with the symptoms, challenges and stresses associated with serious illness. Palliative care might serve you well if:
• You have frequent ER visits or hospital readmissions
• You have been admitted to the hospital to address symptoms of your illness or side effects of its treatment
• You need resources to help cope with the financial, emotional and caregiving stresses associated with serious illness
• You need guidance about care options and how they align with your personal goals
• You are looking for another layer of support in addition to your current medical providers or home health team
• You are coping with cancer, heart disease, lung disease, kidney failure, dementia, HIV/AIDS, ALS, or other diseases
Palliative care services are provided wherever you reside: private home, assisted living community, post-acute care facility, long-term care facility or hospital.
Palliative Care coverage
Palliative care services are covered under Part B of Medicare. We are contracted with many insurance carriers. A member of our care team will contact your insurer to review your benefits and discuss them with you.
