Ascend Palliative offers a variety of services based on a coordinated care plan for each patient with the goal of making you feel better. Our Palliative Care Certified medical director oversees our program. Under their direction a nurse practitioner provides comprehensive evaluation, assessment and coordination with your own doctors and specialists. A social worker may assist in providing resources for practical and emotional support.
Medical Support
The discomfort caused by your illness, as well as the side effects of the treatments often cause symptoms that can be addressed by palliative care. With Ascend Palliative Care, you may experience relief of symptoms including:
Pain
Shortness of breath
Nausea/vomiting
Constipation
Fatigue/insomnia
Loss of appetite
Anxiety and depression
What Does Care Cost?
Hospice care is a 100% covered benefit under Medicare. Medicare covers any needs related to the life-limiting illness including our services, hospice related medications, medical equipment and supplies. Hospice is also covered by many insurance plans.
Medicaid and Medicare often cover palliative care. Medicare and other insurances can be complicated! We can assist you in determining whether you would have out-of-pocket costs, co-pays or other charges.
Where Do I Receive Care?
Care is delivered at home or in homelike setting including nursing homes, assisted living facilities, veterans’ facilities, and hospitals. Ascend can help determine which type of care is most appropriate. Home visits can also be arranged to assess how to best serve the patient and their family.
