Ascend Dementia Program

The Ascend Dementia Program is a specialized hospice program for patients with late-stage or advanced dementia with a strong emphasis on education, assessment and multi-sensory interventions. Our mission is to deliver a personalized approach of comprehensive hospice care and services with integrity and compassion. Our goal is to honor and respect the individual as they prepare for their final journey by providing an individualized plan of care, specific to their dementia needs.

Our approach involves multi-sensory interventions designed to engage the core of the person and promote interaction by incorporating all of the senses: sight, sound, smell, taste and touch. A special care package is designed with each patient and family member in mind. Each customized care package contains supplies to encourage care of the mind, body and spirit to promote quality of life.

Ascend Heart Program

The Ascend Heart Program is a specialized hospice program for patients living with late and end-stage heart failure and cardiac disease. With the Ascend Heart program, we work closely with patients and families, emphasizing comfort in order to relieve the symptoms of heart failure and reduce the re-hospitalization of late and end-stage patients.

Our goal is to minimize hospitalization and maximize quality of life by utilizing the hospice benefits with care provided by a team of trained professionals. A specialized care package is designed to create a better end-of-life experience to relieve symptoms of late and end-stage heart failure.

Veterans Program

The Veterans Program at Ascend Hospice is a special hospice program for our patients who have served our country in the Armed Forces. Our goal is to serve as a source of comfort and support by addressing the unique emotional needs of veterans and their families.

A special recognition ceremony is provided for each veteran. The significance of this ceremony is to acknowledge and validate the service and sacrifice our veterans and their families.

With our strong emphasis on education, Ascend Hospice has trained all staff members on the most commonly found issues that pertain to our veteran population. Community education is also supported and provided to increase awareness of veteran and family needs.

PATH Program

When coping with a serious illness, there are many decisions that may seem overwhelming. The Ascend Health PATH Program is designed to help support patient and their loved ones to help make things more manageable.

Whether a patient is new to us or someone we have been caring for regularly, we understand that there may be times when a decision is made to postpone or stop hospice care services. This does not mean our relationship needs to end. This may be the very time when families have additional questions or concerns that we can help with.

Our trained hospice staff can help provide the guidance needed. Our team will stay in contact with you through this time of transition. Whether it is to discuss changes in an individual’s condition, connecting you with community resources, or simply reaching out to offer encouragement — we will be there.
